Crisis Intervention

Crisis Intervention includes an assessment and Plan of Care but is done in a much more rapid timeline due to the nature of your situation.  Care Manager's are available 24/7 for crisis situations. Urgent/emergent family situations can include the following: 

• Behavioral outbursts
• Wandering
• Hospital or rehabilitation discharge
• Mental health issues
• Family conflict
• Exploitation
• Abuse or neglect
• Self-neglect such as malnutrition, etc.
• Medication mismanagement
• Safety issues
