History

Arizona Care Management was conceived in 2000. The purpose of ACM is to provide a superior level of Care Management services to persons with cognitive impairments, physical disabilities, developmental disabilities and mental illness.

Chief Operating Officer, Heather Frenette, is a Registered Nurse who worked in various aspects of nursing for 20 years prior to starting Arizona Care Management. She prides herself and the company as being a guide, advocate, and resource for families that are caring for a loved one.  We believe that every person deserves a chance to live his or her life at the highest functional level in the least restrictive environment possible.  To ensure this, we work with the client, their family and their current support system to provide services that are tailored to meet the client's specific needs.  It is a delicate balance to maintain the client's independence while providing a safe living environment.

Our Care Management team consists of master prepared staff including a Social Worker and Registered Nurse with the support of a Certified Nursing Assistant. We are members of the National Association of Professional Geriatric Care Managers and are bound by their Pledge of Ethics and Standards of Practice.
