
A Care Manager is a highly qualified individual who has advanced degrees in nursing, social work, gerontology, or a related field. Care Managers are trained to assess, plan, coordinate, monitor, and provide services for the client and their families.
Care Managers have extensive knowledge about the costs, quality, and availability of services in their community. The Care Manager works with the client and the family to provide services that are tailored to their needs. The Care Manager objectively views the decisions to be made, always keeping the best interests and needs of the client first. The services provided by the Care Manager can include, but are not limited to, the following:
Care Management

Comprehensive assessment of the client and the living environment

Develop and implement a personalized Plan of Care that identifies problems, eligibility for assistance, need for services, and preferences of the client and the family
- Crisis management, prevention, and intervention
- Provide preventive services to reduce premature institutionalization and overuse of services
- Coordinate medical care and recommendations for referrals to appropriate specialists
- Act as an advocate for the client
- Intervention strategies to help families with difficult issues
- Identify the most viable options, resources, and support services to optimize care and provide for the health, safety, well-being, and autonomy of the client
- Short-term or on-going monitoring, assessment, intervention, and revision of the Plan of Care
- Professionals on-call 24 hours a day, 7 days a week
- Review of current medication
- Medication set-up by a nurse
- Coordination of transportation
- Offer referrals to legal and financial professionals
- On-going communication with families and support systems
- Assistance with relocation to alternative housing
- Assistance with moving the client across the country to be closer to family and support systems
- Household financial assistance
Homemaker/Companion and Non-Medical Caregiver

Our caregivers are all employees, not independent contractors. Our rates include worker's compensation, social security, unemployment, and all employment taxes. The caregivers are insured through our company and also are required to carry their own private automobile insurance. All of our caregivers are fully screened to ensure your loved one is receiving the highest quality care possible. Each caregiver is background checked (including criminal, DMV, and social security), fingerprinted, and drug screened. In addition, each caregiver has completed CPR and first aide classes.
