
Heather Frenette, RN, MSN, CMC - Chief Operating Officer & Care Manager
Ms. Frenette is a Registered Nurse and Certified Private Fiduciary who focuses her practice on the care needs of elderly, disabled, and mentally ill persons. She received her Bachelors of Science in Nursing from Baylor University, a Masters of Science in Nursing from The University of Texas at Arlington, and a Masters level Certificate in Gerontology from Arizona State University. Ms. Frenette is also a Certified Care Manager (CMC) through the National Academy of Certified Care Managers. Ms. Frenette is actively involved in the Western Region National Association of Professional Geriatric Care Managers as both a Board Member and an Officer since 2001. In 2006, Ms. Frenette was recognized as a Fellow of the Leadership Academy, a distinction awarded to those geriatric care managers with comprehensive experience. She has also received the Rose Kleiner Award for excellence and achievement in the field of Professional Geriatric Care Management. As an advocate for the elderly, disabled, and mentally ill, Ms. Frenette believes that every person deserves a chance to live his/her life at the highest functional level in the least restrictive environment possible. Ms. Frenette is a member of the National Association of Professional Geriatric Care Managers, the Arizona Fiduciary Association, the National Guardianship Association, the Arizona Geriatrics Society and an Associate Member of the Arizona State Bar.
Graham Thema, CQSW, MSW, ASW - Care Manager
Mr. Thema has been with Arizona Care Management since 2001. He is a Masters prepared social worker with a liberal arts background. His career spans two decades and two countries, Great Britain and the United States. Born in England, he graduated with his Masters degree jointly from Nene College in Britain and the University of Wisconsin. He is also a Certified Care Manager (CMC) through the National Academy of Certified Care Managers. Mr. Thema was further awarded his ASW, (approved social worker under the mental health act of Great Britain), the equivalent of the United States LCSW. Mr. Thema has extensive experience in crisis intervention, mental health issues, and family counseling. Upon moving to the United States in the late 1980's, Mr. Thema worked as a hospital social worker covering the Emergency Room with additional responsibilities in oncology and home health. Mr. Thema is a member of the National Association of Social Workers and the National Association of Professional Geriatric Care Managers.
Adam Will, MA - Care Manager
Mr. Will joined Arizona Care Management in 2008. He has over a decade of experience in Geriatric Care Management, providing social services to the chronically ill and disabled. Adam has a strong background in Medicare and Medicaid and has also worked in the geriatric psychiatric setting helping patients and families cope with the challenging issues presented by mental illness, Alzheimer's, and dementia. A native of Arizona, Adam holds a Masters in Counseling from City University of Washington with a Bachelor of Arts degree from Arizona State University. He is also a Certified Care Manager (CMC) through the National Academy of Certified Care Managers. Mr. Will is a member of the National Association of Professional Geriatric Care Managers.
Debra Strecker, CNA - Care Management Assistant
Ms. Strecker has been with Arizona Care Management since 2000. She is a Certified Nursing Assistant (CNA) who is currently working towards a nursing degree. She has been working with the geriatric population since 1994.
Tianna Wahl- Care Management Associate
Ms. Wahl joined Arizona Care Management in 2008. She has worked in various clerical and financial roles; including banking, accounting, and administrative support positions. She obtained her Associates Degree from Pima Community College in Tucson, Arizona.
Suzette Armijo- Community Outreach Liaison
Ms. Armijo began with Arizona Care Management in 2009. She came from the housing industry and is implementing a Community Outreach department to help educate the community about our services. Suzette brings over 10 years marketing, advertising and public relations experience to our team. Suzette is a member of the National Association of Professional Women and the National Private Duty Association.
In addition, we provide Guardianship, Conservatorship, Trustee, Personal Representative and POA services through The Sun Valley Group, Inc. Our Care Management staff works closely with our Fiduciary staff to provide full service Care Management and Fiduciary Services. Arizona Care Management is a division of The Sun Valley Group, Inc.
